Jack Cook, 82, of Paducah died Tuesday at Lourdes hospital.
Survivors include his wife, Beverly Cook; four nephews; and two nieces.
He was preceded in death by his parents, Alva and Maggie Cook, and five sisters.
A memorial service will be held at noon Saturday at Mr. Cooks home with Larry Walker officiating. Lindsey Funeral Home is in charge of arrangements.
