Jack "AY" Simmons

Jack “AY” Simmons, 81 of Mayfield, passed away Wednesday August 8, 2018 at his home.  Jack was avid  traveler and photographer.

He is survived by his wife, Elaine Simmons of Mayfield.  One daughter, Darla Evitts of Madisonville, KY.  Two sons, Randy of Paducah, and Larry Simmons of Mayfield, KY.  One sister, Patricia Winfrey of Hickory, KY.  7 grandchildren and 2 great grandchildren.

He was preceded in death by his parents Elzie and Hilda Simmons.  And two brothers, Don and Frank Simmons.

Private family services are scheduled at later day.
